4,000 of this card were issued in a special Telecom Presentation Folder, illustrated below bottom, and distributed free to every Telecom Phonecard standing order customer,
by including it in the McDonalds Collector Pack, see NZ-CP-42. These cards had Collectors Issue 1995 on the reverse and this same card was also included in the International Pack,
i.e. there was not an International version.

This card was not included in any other Collector Pack.
